This program may involve instruction in various disciplines of yoga, meditation and other movement exercises such as Pilates, exercises, breath and meditation practices, hydrotherapy (i.e. the use of the saunas, steam rooms, hot and cold baths), vegetarian diet, intensive schedule, or other physical exercise such as skating, walking, hiking up hills and trails, and using exercise equipment.
I acknowledge that photos and videos will be taken . I release use of my image for marketing purposes.
I acknowledge that I have no medical condition that might render me unfit to perform such activities, and that I am free to seek medical or any other advice at any time prior to or at any time during the course. I wish to undertake this activity of my own free will.
I understand that Sacred Roots Holistic Healing Center LLC and Estro Den Ranch is not engaged in the diagnosis treatment or cure of any disease.
I understand that to the extent that I take any prescription or over the counter medication Sacred Roots Holistic Healing LLC and Estro Den Ranch advises that you seek medical advice prior to undertaking this program .
I also understand that no illegal substances are allowed on the premises, and that possession of such substances will result in being asked immediately to leave.
I accept full responsibility for my health, monitoring of my own physical condition at all times.
I will communicate with the staff should I experience any difficulty or discomfort when participating in any of the aforementioned activities.
I acknowledge that I am free to discontinue any activity at any time.
I acknowledge that I have been given the opportunity to inform myself fully and I hold Sacred Roots Holistic Healing Center LLC and Estro Den Ranch harmless for any health condition that may arise in connection with any of the activities associated with this program.
I understand that the location in which these activities may be undertaken include but are not limited to a remote forested environment, rocks along the paths, slippery and or decomposed granite flooring, slippery grass flooring, loose surfaces on boulders, dry and wet creeks and ponds, decks that may be slippery, construction areas, woodpiles, poisonous snakes, bears, wildlife, and other natural and man made, physical and structural conditions, and I accept full responsibility for exercising caution and prudence in such environment.
I release and hold harmless Sacred Roots Holistic Healing Center LLC and Estro Den Ranch for any injuries, to the extent that such release permitted by law.
I hereby freely and voluntarily assume any and all risk including any injury or physical discomfort, loss or damage associated with my participation in such activities.
I agree to protect, indemnify and hold harmless Sacred Roots Holistic Healing LLC and Estro Den Ranch, their officers, staff, instructors, representatives, agents, employees, and assigns against any and all damages, claims, suits, judgments, fees, costs, and other expenses, arising out of this agreement or any activity undertaken with reference to this agreement.
I have carefully read and fully understand this agreement and release, and accept the stated terms and conditions.
I understand that I may consult an attorney, medical provider or any other person at any time before or after signing this agreement.
I understand that any cancellations within 7 days of retreat are only eligible for a 50% cash refund.
